Provide all pertinent information, including the trainer’s name, the card issuing date, the location of training, the type of training (e.g. construction, general industry) and whether it is a 10 or 30-hour card. Include any other relevant information including your name and contact information. OSHA will maintain the confidentiality of the person reporting the information.
