For cards that were issued prior to March 2016, please contact our office at 866-936-6742 or contact us online. For cards that were issued March 2016 and after, you can log in to your account in our outreach portal and select “Request Replacement.”

Please note:

  • Replacement cards cannot be issued if the original training took place more than five (5) years ago.
  • Each student may only receive one (1) replacement card.
