Human and Organizational Performance (HOP) offers a different lens: one that looks at the conditions that shape how work actually happens, not just how it was designed to happen.

Error is not a character flaw. It is a predictable by-product of complex systems. HOP shifts the fundamental question from “who made the mistake?” to “what conditions made this outcome likely — and what can we learn from that?” This shift changes everything: how you investigate incidents, how leaders engage with their teams, how you design work, and how you measure what actually matters.
