Safety programs built on rules, procedures, and compliance are necessary. But they don’t explain why incidents keep recurring, why workers find workarounds, or why corrective actions don’t stick. Human and Organizational Performance (HOP) offers a different lens: one that looks at the conditions that shape how work actually happens, not just how it was designed to happen.
Error is not a character flaw. It is a predictable by-product of complex systems. HOP shifts the fundamental question from “who made the mistake?” to “what conditions made this outcome likely — and what can we learn from that?” This shift changes everything: how you investigate incidents, how leaders engage with their teams, how you design work, and how you measure what actually matters.
This is not a lecture. Participants bring their own incident reports, safety metrics, permits, and procedures — and leave with practical tools, a gap assessment, and a roadmap for integration into their own organization.
You will learn how to:

· Recognize the difference between work-as-imagined and work-as-done, and why that gap is your greatest source of learning
· Move beyond “root cause” to systems-based investigation and operational learning
